In Albania, according to the National Environmental Agency, 16 companies have environmental permits for the treatment or transportation of hazardous hospital waste.
However, only four of them regularly win tenders from major hospitals: Eco Riciklim, VALE Recycling, Medi-Tel, and Pura Medical. Referring to treasury transactions for the period 2019 -2025, the company Eko Riciklim has benefited from about 614 million new lek for hospital waste management, or about 6 million euros.
Data shows that various hospitals in the country have contracted this company, including the Trauma Hospital, dental clinics, but also primary health care branches.
The largest number of tenders, 13, this company has benefited from the "Xhafer Kongoli" Hospital Center in Elbasan, 10 tenders have been received from the trauma hospital, 9 from the "Queen Geraldine" Obstetric-Gynecological University Hospital, Tirana and 8 from the "Mother Teresa" University Hospital Center. Regional hospitals in the country are listed below.
"The first alarm is, the village that seems to want no leader at the moment we have seen how large piles of hospital waste appear beyond the walls, so they have exceeded the height of the walls, we can see them with the naked eye and they are not being destroyed, the problem is major," says infectious disease doctor, Ilir Alimehmeti.
If we refer to Open Data data, out of 77 tenders that the Eco Recycling company has benefited from, 48 tenders are red flagged. That is, with a high risk of corruption or lack of competition.
The highest value tenders are part of this list. The tender for the treatment and disposal of hospital waste, awarded by the QSUT to this company, has a term of 24 months and a value of 142 million lek. The tender was carried out on February 20, 2025. This tender lacked competition as there was only one operator in the competition.
The other tender worth 138 million lek related to this company from the trauma hospital for a period of 48 months again results in a lack of competition. From a check at the Central Bank it results that this company is managed by Ervin Meçe who is also a partner with 30% while the company "ANA Mark" is a partner with 70%.
"Ana Mark" is a company owned by Klodjan Allajbeu, the owner of the private network called Spitali Amerikan. Data suggests that this network of companies has had connections with the company Eco Riciklim in the past, before the official sale of quotas took place in 2025.
Regarding the value of the benefits from the tenders for the treatment of hospital waste, Medi-tel has benefited from 28 million new lek. In total, it has benefited from 121 tenders. QSUT leads the list with 14 tenders, followed by the “Shefqet Ndroqi” Hospital with 13 such, the National Blood Transfusion Center and the Durrës Regional Hospital have 12 tenders each, respectively.
46 tenders have been listed with a red flag due to lack of competition or disqualification of other entities. The tender with the Shkodra regional hospital in 2022 with a limit fund value of 2.3 million lek excluding VAT was won by this company being the only one in the competition. Another tender in 2022 with a 1-year term and a value of 2 million lek was considered problematic due to the disqualification of other competitors.
"We have seen another company that was prosecuted for an incinerator that took waste, but this incinerator never existed, it was never purchased. There is no legal body that sees this, it is definitely not invisible," says doctor Alimehmeti.
The VALE RECYCLING company, owned by Elvis Miri, was at the center of a scandal. It won dozens of tenders for the treatment of hospital waste even though it did not possess the necessary technological line. In the procedures, it presented false documents as if it had them.
It is suspected that it has won at least 30 tenders in this form worth about 65 million new lekë. The company has claimed possession of the Econos Tsc 150 device, for the treatment of hospital waste. "VALE" has declared this device since 2016, but has only purchased it at the beginning of 2024. Currently, this case is under trial and has the administrator of the VALE entity and 10 other people under indictment.
Despite the scandal, this company has collected millions throughout 2025, while the administrator is facing justice for tender abuses. From 2019 to 2025, the entity has collected from the treasury a full 487 million new lekë, or approximately 4.9 million euros.
The company has won a total of 72 public contracts from various hospitals across the country. Of all the tenders won by this company, 21 are red flagged, meaning the procedure was questionable, with a high risk of corruption.
"All those who close an eye, close an ear, close their mouths, who do not speak, do not hear, do not see, are all part, some more and some less, of this great problem. First of all, we must admit that the problem exists and we have this in official reports, we have it in the media, we have it clearly before our eyes and we see it," the doctor continues.
The Pura Medical company has benefited from the state budget in the period 2019-2025 by nearly 50 million new lekë. This company has benefited from 12 tenders for hospital waste alone from the QSUT, the “Shefqet Ndroqi” hospital, the Durrës Regional Hospital, the Korçë Hospital, etc. Two tenders of this company have been listed with a red flag. One procurement with the Saranda hospital and another with the Berat regional hospital.
The National Environmental Agency says it has carried out inspections of relevant entities dealing with waste treatment and in some cases has also taken administrative measures, such as fines.
"Medi Tel, it turns out, was inspected 3 times from 2023-2025. VALE Recycling, it turns out, was inspected 11 times from 2021-2025 and it turns out that there was 1 administrative measure of fine and suspension of activity in 2023.
"Eco Riciklim, it turns out to have been inspected 10 times from 2021-2025 and it turns out to have no administrative measure.
