"MALDI-TOF", a must for microbiology laboratories!

MALDI-TOF - during this decade, it has become an indispensable tool for clinical microbiology laboratories.

Ø Identification speed

Ø Accuracy,

Ø Low cost

Ø Waste reduction is a characteristic of this technology.

There is also tremendous potential for antibiotic susceptibility testing (AST) and subspecies analysis.

MALDI stands for "Matrix Assisted Laser Desorption/Ionization".

TOF MS is short for time-of-flight mass spectrometry

MALDI-TOF is one of the most popular MS instruments used for its rapid and accurate genus and species identification of a wide range of gram-negative and -positive bacteria. Microorganism identification by mass spectrometry is based on identifying a characteristic spectrum of each species and then matching it to a large database within the instrument. The current review provides a contemporary perspective on the challenges and opportunities for bacterial identification, as well as a written account of how technological innovation has advanced MS.

Future clinical applications will also be addressed, particularly the use of MALDI-TOF MS in the field of microbiology for the identification and analysis of antibiotic resistance.

Ø  MALDI-TOF MS is an emerging technology and has recently been used to identify microorganisms.

Ø It is a high performance, cost effective method with a high degree of accuracy. This is particularly important for routine clinical microbiology, as results can come directly from positive BC (blood culture), urine or other biological fluids, as well as subcultures on agar plates and broth media.

Ø The reliability and accuracy of MALDI-TOF MS have been verified in a number of studies, and the accuracy of identification of microorganisms depends critically on the number of entries in the database. Therefore, continuous updating of the reference database is necessary for performance advancement.

Ø Further database improvements in the MALDI-TOF MS system allow rapid identification of antibiotic resistance characteristics and reliability, as well as direct identification of pathogens in urine and BC samples.

Ø For filamentous fungi, Becker et al. showed that MALDI-TOF MS correctly identified 95.4% of isolates to the species level Gautier et al. examined the identification of clinical mold isolates from patients with aspergillosis, fusariosis, and mucormycosis.

Ø Continuous expansion of the library is essential to maintain the increased reliability of MS in the future.
