The Mandatory Health Insurance Fund (FSDKSH) continues to be at the center of a series of problems identified through investigations, audits and complaints related to drug reimbursement, contractual relations with pharmaceutical companies and the control of warehouses and pharmacies.
The behind-the-scenes media has published a denunciation article where the investigation focuses on the current director of FSDKSH, Ilir Topi, the former directors of the institution, the company "CFO Pharma", as well as the findings of the Supreme State Audit Office (SSA) and the investigations of the Tirana Prosecutor's Office and SPAK.
Ilir Topi's asset declaration
According to the material, the director of FSDKSH, Ilir Topi, has declared a three-story apartment with a total area of 223.4 square meters and a construction area of 128 square meters per floor, worth around 35 thousand euros.
Topi has declared income from emigration, agricultural activity, and savings generated from salaries.
According to the document, there are inconsistencies in his statements regarding the period of his stay in Greece.
Ilir Topi, who previously worked as a customs officer in Korça and a tax officer in Tirana, completed the decriminalization form in March 2021, as part of his mandate as a Socialist Party MP.

In the decriminalization form, he does not declare that he lived abroad for immigration reasons. Topi declares that during the years 1995-1997 he stayed in Crete, Greece for studies and not as an immigrant.
Meanwhile, his CV as a member of parliament states that during the period 1995-1996 he was in Greece for postgraduate studies at the Mediterranean Agronomic Institute, Advanced Studies, in Maich, Crete.

These discrepancies between the CV, the decriminalization form, and the asset declaration, according to the material, raise questions and suspicions that require verification by the competent institutions.
Contract with “CFO Pharma”
One of the main issues mentioned in the material is related to the company "CFO Pharma", owned by Nadir Çausholli.
According to the material, in September last year, the former director of FSDKSH, Spartak Zekja, was arrested by order of the Tirana Prosecutor's Office as part of investigations into a suspected fraud scheme and illegal profit from drug reimbursement funds.
The investigations, according to the material, also affected other Fund officials and pharmaceutical entities.
In this context, the material raises the claim that the problems identified earlier may have continued even after the change in the management of the institution.
According to the document, Ilir Topi has signed the reinstatement of the contract with "CFO Pharma", a company that was previously involved in investigations for a suspected fraudulent scheme with the reimbursement of medications, worth 700 thousand euros.
Mes tyre përmenden Astrit Beci, Milva Ekonomi, Vjollca Braho, Erion Manohasa, Alketa Hila, Eduard Ahmeti, Anila Denaj, Adela Xhemali, Rudina Dergjoni, Elisa Bërziu, Albana Çela, Spartak Zekja, Arti Papajani dhe drejtori aktual Ilir Topi.

According to the report cited in the material, this deficiency made it impossible to objectively assess the effective availability of medicines and fulfill contractual requirements.
The Albanian Supreme Audit Office considers the failure to document the expiration date a fundamental deficiency, as it affects the transparency, accuracy and reliability of drug inventory control.
Spartak Zekja and the investigation into drugs
From September 2023 to September 2025, FSDKSH is led by Spartak Zekja.
According to the material, Zekja was previously associated with the pharmaceutical sector and was the founder of the company Distributor of Medical Devices Group sh.pk, which also had a pharmaceutical warehouse.
To avoid a conflict of interest while running the Fund, he had transferred the company's shares to Matteo Shehu. The document, however, raises allegations of his continued ties to the pharmaceutical sector.

In November 2025, the Tirana Prosecution Office arrested Spartak Zekja along with other individuals.
According to the material, investigations uncovered a suspected fraud scheme with fictitious inventories and invoices for the drugs "Foster" and "Trimbow", through which it was suspected that reimbursement funds were obtained from FSDKSH.
At the center of the investigation was "CFO Pharma", along with the entities "Pharma Plus" and "Medix Albania" and persons connected to the Fund's structures.
Albana Çela and the management of the Fund
Also the former director of FSDKSH, Albana Çela, who led the institution during the period 2020-2023. According to the document, the KLSH recommended her dismissal. In September 2023, she was replaced by Spartak Zekja.
Albana Çela (Adhami) is a close associate of Ogerta Manastirliu, former Minister of Health, with a file in SPAK on health affairs. The directors of the Fund were dictated by the oligarchs through Manastirliu, who was the chairwoman of the Administrative Council of the Fund for the period 2017-2023, in the capacity of Minister of Health./Taken from Prapaskena
